Streamlining the eNRMC medication chart process for Fronditha Care

Solution

Telstra Health developed MedPoint within our well-proven and award winning Medication Management solution to help aged care facilities, prescribers (such as GPs and Nurse Practitioners) and pharmacists easily manage the eNRMC process. MedPoint does not require separate licensing costs for Medication Management clients like Fronditha, meaning providers can put their resources into what matters most – delivering quality care to residents.

Fronditha Care is a five-facility non-profit aged care provider with cultural ties to the Victorian and New South Wales Greek communities.

The transformative impact MedPoint has had on Fronditha Care’s medication charting process.

Following the Federal Government’s introduction of electronic National Residential Medication Charts (eNRMC) in residential aged care, Fronditha Care took the opportunity to adopt Telstra Health’s eNRMC conformant module, MedPoint.

Introducing MedPoint

With MedPoint, prescribers can create and update a resident’s medication chart electronically – both from within the facility or remotely – which decreases the potential risks associated with hand-written charts. The electronic record then flows automatically to the pharmacist, ensuring the timely provision of medications and reducing the manual process of scanning and emailing charts for facility staff.

Previously, Fronditha Care faced challenges with the manual process of paper-based medication charts, GP prescriptions, and pharmacy communication.

Mr Rodrigo elaborates, “With paper-based systems, different review dates made it difficult to stay on top of everything. MedPoint streamlines this process and allows for automatic communication with pharmacies, eliminating the need for our staff to fax, scan or email medication charts. This not only helped cut down our staff time, it has also improved the timeliness of medication delivery.”

One source of truth for clinical and medication data

MedPoint is part of a wider Medication Management solution, including the award-winning MedMobile app, which assists providers with the safe administration of over half a million medications on a daily basis across Australia. For added risk reduction, information captured in MedPoint and MedMobile seamlessly forms part of the resident record in Telstra Health’s clinical and care management solution, Clinical Manager.

This seamless experience helps reduce the risks associated with resident medication or allergy information needing to be manually added into two separate systems – helping to protect our older community.

“For us, having one system that would manage our medication and clinical data was our priority”, said Mr Rodrigo. “Our Board recognised the importance of a wholistic system so that all our resident data is in the one place.”

Tailored implementation and support

The Telstra Health team have provided ongoing support both to Fronditha Care and their partner prescribers and pharmacists.

Understanding the time constraints many prescribers and pharmacists face, the Telstra Health team provided online training sessions, as well as access to a comprehensive Learning Portal containing written and video guides.

Mr Rodrigo stated, “The Telstra Health team has been helpful from the beginning. The training sessions were well-received by our staff. The Telstra Health team even offered one-on-one training sessions to our GPs to ensure they would be comfortable with the transition to the eNRMC.”

The implementation process for Fronditha is tailored for each facility, and consists of project preparation, a two-week implementation, followed by a period of hypercare to ensure a smooth transition once the solution has gone live.

Reflecting on the key benefits of MedPoint, Mr Rodrigo emphasised, “The MedPoint module streamlines everything, from GP communication to pharmacy processes. It eliminates the need for paper charts, making medication management safer and more efficient. It also maximises the use of telehealth consultations, allowing for timely interventions.”
